1.Police Report/Driver’s Notarized Affidavit/Incident Report (FPG Insurance’s Traffic Accident Situation Form)
2.Driver’s License with its official receipt
3.Updated Certificate of Registration with its official receipt
4.Photos of the vehicle’s damage and its plate number
5.Repair Estimate issued by your preferred shop
Theft Claims (Accessories and Carnap)
1.Police Report (for theft of accessories only)
2.Repair Estimate issued by your preferred shop (for theft of accessories only)
3.Certified true copy of Police Report (for carnap cases)
4.Certified true copy of Alarm Sheet (for carnap cases)
5.Certified true copy of Complaint Sheet (for carnap cases)
6.Original Certificate of Non-Recovery (for carnap cases; to be issued by Camp Crame)
Total Loss Claims (on top of the requirements for Own Damage/Theft)
1.Car Keys (original and duplicate)
2.Original Copy of the Certificate of Registration with OR
3.Original Copy of the Signed Deed of Sale (if applicable)
4.Original Copy of the Signed Secretary Certificate (for juridical entities)

Third Party Bodily Injury Liability
1.Original Hospital and/or Doctor’s Medical Certificate
2.Original Hospital Bills/Invoices and OR
3.Original Medicine Bills/Receipts and OR
Third Party Property Damage Liability
1.Original Certificate of No Claim to be issued by the insurer of the third party vehicle owner
2.Third Party Driver’s License with its official receipt
3.Third Party’s Certificate of Registration with its official receipt
4.Photos of the damaged third party vehicle and its plate number
5.Third Party’s repair estimate
CTPL (Compulsory Third Party Liability) is mandatory for vehicle registration and covers third-party bodily injury or death. Comprehensive Insurance includes CTPL and adds protection for your own vehicle against theft, collision, natural disasters, and more. This is obtained separately from the CTPL Policy.
Yes, but some insurers may require repairs to be done at accredited shops. Let us know your preference and we’ll coordinate accordingly.
Standard Process
Own Damage claims
1.Report the claim as soon as possible by contacting LLIBI.
2.Email to LLIBI the documents required, including driver’s license, vehicle OR/CR, and the incident report. We will inform you if any other documents are required.
3.LLIBI to arrange inspection via IMSI, an independent third party appraiser.
4.IMSI may also perform the inspection at your selected motor shop, including dealer or 4-star shops.
5.LOAs shall be provided to the shop for the repairs based on IMSI’s evaluation. (IMSI sees to it that the repair costs are fully covered.)
6.Pay the deductible to the shop (PHP 2,000 for private cars and PHP 3,000 for commercial vehicles) and any depreciation for replaced parts, if applicable.
Third Party Liability Claims Procedure
1.Report the claim as soon as possible by contacting LLIBI.
2.Provide the name, contact details, and vehicle information of the third party to LLIBI.
3.Email to LLIBI the documents required, including driver’s license, vehicle OR/CR, incident report, and the Police Report. We will inform you if any other documents are required.
4.LLIBI shall assign an adjuster to deal with the third party. The adjuster may handle direct dealings and communication especially if the third party is adverse and uncooperative. Do not in any case admit liability.
5.Do not settle directly with the third party, noting that any settlement, so that it may be covered, needs to be pre-approved by the insurer.
