This policy is arranged and serviced by Lacson & Lacson Insurance Brokers, Inc., who may assist in respect of any inquiries relating to coverage and claims.

| CLAIMS GUIDELINES |
Submit your claim to Lacson & Lacson Insurance Brokers, Inc. All claims are reviewed, processed, and handled for you by Lacson & Lacson Insurance Brokers, Inc.
- Claims should be notified via phone call or e-mail to Lacson & Lacson Insurance Brokers, Inc. within 30 days from the date of accident, except death cases, which should be notified immediately.
- Submit a duly-accomplished Personal Accident Claim Form (see back) and the corresponding Claim Document Requirements (also in back) to Lacson & Lacson Insurance Brokers, Inc. within 90 days from the date of accident. Claim documents may be submitted in advance by e-mail; original documents should be submitted directly to Lacson & Lacson Insurance Brokers (Attention: Randy dela Cruz), 15/F Burgundy Corporate Tower, 252 Senator Gil Puyat Ave., Makati City OR may be left at the APSA Office, for us to pick up every Tuesday and Friday.
- We will notify you when your claim check is available. If your documents are complete, our standard processing time is 7 to 10 working days.
